Am I blind or has Kedge Keep disappeared, at least on Nightmare difficulty?

Not seeing it on Hell.

On the other hand, I see it on Normal.

Also, a couple of minor nitpicks:

1. Solusek's Eye is listed after South Ro and South Karana.
2. Nektulos Forest appears after Neriak.

Just finished the main quest on normal with my ranger. Seams Fear is not repeatable, but hate is.

Also, after finishing Hate. It shows the quest as being finished. If I try to pull a mob I get the message "The zone has been cleared."
Guessing I have to log out and back in to do it again.

Fear not repeatable? Anyone else see this problem? Seems pretty repeatable to me And, yes, the zone cleared message happens in every boss zone after clearing it.

Both give the normal "zone clear" message if you don't log out. Both repeatable as normal if you do log out. As of 2 minutes ago tested both ways/
