So I've just run my cleric through Act 1 and I noticed that at some point her auto attack was no longer counting down. This would happen mid-chain so that I checked that on pull it started counting down and then happily nuked my way through orcs and things (using 1 and D on the keyboard and mouse for other skills) and at some point during the chain, there would no longer be a countdown on the auto attack button. I've checked my options which has auto attack on.

Any ideas? I know that particularly through the last sections of Act 1, I was making sure not to fat finger 'A'.

Is auto attack on? It can be turned off automatically by root. "A" turns auto attack on. If the monsters name isn't red, auto attack was turned off.

Yeah, root disables auto attack. It tries to help you out in the same way Rogue's Flash Powder disables your auto attack.

Next time I run her, I'll check =)
